Below are the various forms needed by committees and individuals who volunteer for the Ohio-Meadville District. If you need a form and can't download it or find it here, please contact the OMD office.
Expense Forms are available in three different formats - word, excel and pdf. The excel and pdf forms will calculate the mileage rate and expense totals for you if you fill them out before printing. Also, if you are only reporting mileage, you can submit your form by email since no receipt is required.
