OMD Directory Update Form

It is now time to update your congregational information for the 2010-11 OMD District Directory.

Deadline for updates is July 30th at midnight. Changes submitted after that date will be updated as time permits.

If you have already completed the survey and discovered that you need to update or change any of that information, please just email it to the district office. Our program is currently unable to allow you to go back and make changes after the survey is completed.

Please note that while you may have already provided this information to the UUA, we need it too and not all the information we ask for is the same. Please update your directory information as soon as your leadership for the year is set. This includes committee chairs.

You need only update what has changed. Please check your congregation's page for the 2009-10 directory to see what needs updated. Copies are available at each congregation or you can contact the office to request one.
