OMD Curriculum Fair Registration

Please complete the following form to register for the OMD Curriculum Fair in North Olmsted, OH on March 14, 2009. Clicking submit will register your information. You will receive a confirmation email.

There is a $10 per person charge for all adult attendees for lunch. This fee can be paid onsite. If you wish to pay be credit card, select that option. Credit card payments are through Paypal and you will receive a second confirmation email for your credit card transaction. If you wish to pay for more than one person on a credit card, please email the OMD Office for instructions (do not include your credit card information).
