You will receive an confirmation email when you complete the form.

Credit card payments are through Paypal and you will receive a second confirmation email for your credit card transaction. If you have a paypal account you can also choose to debit your checking account. You can mail a check to the district after you are registered online or send a check from your church. If you wish to pay for more than one person on a credit card, please email the OMD Office for instructions (do not include your credit card information).

Please note, if you are a youth, you must contact Karen Lapidus to receive the permission and sponsorship forms. Email her at klapidus at ohiomeadville.org
